I don't know about 2002, but I don't have to do anything special under 2000 or 2003. The icon just appears in the list.

Even with the Minimal sample from GapiDraw 2.04, I don't see it appear in the recent applications list.

The last thing I see is the File Explorer icon, from which I launched Minimal.

Actually, I've never seen a GapiDraw icon in the recent applications list. Yet I know this works for GapiDraw applications.

So what must I do to get this to work? What's the trick?

Is it a configuration setting? A return code? An API call?

The app must be launched from the Start menu, Programs or a sub-folder. If you're invoking the app in other ways (e.g. via the debugger, from File Explorer, etc) it will not end up in the MRU.

In other words, placement in the shell's Start menu's MRU is dependent on the app being launched from the shell itself.
